We are growing and looking for an experienced Geriatric Care Manager who is passionate about advocacy, care coordination and better outcomes for older adults.

The Role

The Geriatric Care Manager is a trusted guide for clients and families. You will assess needs, coordinate care and make sure every service is delivered with excellence and compassion.

This role blends clinical judgment, problem-solving and relationship-building. You will work closely with our Care Managers, nursing team and Care Professionals in a fast-paced, deeply meaningful environment.

What You'll Do

CLIENT & FAMILY SUPPORT

• Complete comprehensive in-home assessments and ongoing reassessments
• Build and update individualized care plans
• Serve as a primary point of contact for clients and families
• Offer education, guidance and emotional support through transitions of care

CARE COORDINATION

• Partner with Care Professionals, nurses, physicians, hospice and community providers
• Monitor quality of care and step in early when issues arise
• Coordinate medication support, safety planning and disease-specific care, including Alzheimer's, dementia and Parkinson's

TEAM COLLABORATION

• Coach Care Professionals with client-specific guidance and education
• Share changes in client condition with scheduling, nursing and leadership
• Lead or join care conferences and interdisciplinary meetings

DOCUMENTATION & COMPLIANCE

• Keep accurate, timely documentation in our care management system
• Ensure services meet Colorado regulations and Home Instead standards
• Support quality assurance and outcome tracking

What You Bring

REQUIRED

• Bachelor's degree in Social Work, Nursing, Gerontology or a related field
• 3–5 years of experience working with older adults
• Strong knowledge of aging, chronic disease, dementia and care transitions
• Excellent communication, organization and problem-solving skills
• Valid driver's license, reliable transportation and ability to travel across the Denver metro

PREFERRED

• Experience in home care, home health, hospice or geriatric care management
• Familiarity with Denver-area community resources and healthcare systems
