Position Overview: Home Instead is seeking a compassionate and dedicated Medical Social Worker to join our team in Denver, Colorado. In this role, you will provide essential support and assistance to individuals and families receiving home care services. The ideal candidate will have a strong background in social work, with a focus on medical and healthcare settings. Your primary responsibility will be to assess the psychosocial needs of clients and collaborate with interdisciplinary teams to develop comprehensive care plans that address these needs effectively.

Responsibilities:

1. Conduct comprehensive psychosocial assessments of clients receiving home care services, including evaluating emotional, social, financial, and environmental factors impacting their well-being.
2. Develop individualized care plans in collaboration with clients, families, caregivers, and healthcare professionals to address identified psychosocial needs and promote holistic health and wellness.
3. Provide counseling, emotional support, and crisis intervention to clients and families facing challenging medical diagnoses, life transitions, and end-of-life care.
4. Advocate for clients' rights, autonomy, and access to resources within the healthcare system, community, and social services networks.
5. Collaborate with healthcare professionals, including physicians, nurses, therapists, and case managers, to coordinate care and ensure continuity of services.
6. Educate clients, families, and caregivers about available community resources, support services, and advance care planning options to enhance their quality of life and promote self-empowerment.
7. Maintain accurate and timely documentation of client assessments, care plans, interventions, and progress notes in accordance with regulatory standards and organizational policies.
8. Participate in interdisciplinary team meetings, case conferences, and staff trainings to facilitate communication, teamwork, and professional development.

Qualifications:

1. Master's degree in Social Work from an accredited institution.
2. Current licensure or eligibility for licensure as a Clinical Social Worker (LCSW) in the state of Colorado.
3. Minimum of two years of professional experience in medical or healthcare social work, preferably in home care or hospice settings.
4. Demonstrated knowledge of medical terminology, healthcare systems, and community resources related to aging, chronic illness, disability, and end-of-life care.
5. Strong assessment, counseling, crisis intervention, and care coordination skills.
6. Excellent communication, interpersonal, and collaboration abilities.
7. Ability to work independently, prioritize tasks, and adapt to changing priorities in a fast-paced environment.
8. Proficiency in electronic health records (EHR) and Microsoft Office applications.
9. Valid driver's license, reliable transportation, and willingness to travel to client homes throughout the Denver metropolitan area as needed.
